As the Community Mental Health Center for South Central Kentucky, LifeSkills receives State General Funds (SGF) to provide a variety of supports and services to persons with intellectual disabilities and families living in this region (Region IV).

Regional Case Managers

Regional Case Managers serve as the point of intake, and assist people and their families in identifying and obtaining needed supports. They assist with obtaining medical, clinical, residential, employment, and other services. Case Managers work closely with the person and family to ensure that supports are in place that will assist the person to achieve his or her personal goals.

Crisis Prevention & Response

Crisis Prevention & Response is available to assist an individual and/or family experiencing crisis that may result in the person having to leave his or her home. Crisis services may involve intensive planning and intervention, or may be limited to purchasing special equipment that will enable the person to continue living at home.

Pre-Admission Screening and Resident Review (PASRR)

Pre-Admission Screening and Resident Review (PASRR) involves screening and assessment of persons with intellectual disability, developmental disability, or a related condition to determine appropriateness for nursing home placement. PASRR Specialized Services are provided to nursing facility residents with intellectual disability or other developmental disabilities who have been identified as needing services above and beyond what a nursing facility typically provides. Services are based on the individual’s specific needs and may include supports for participating in day programs or other community activities, adaptive equipment and assistive technology.
